Family Dentistry of Columbus provides comprehensive dental treatment to our patients in a friendly and caring environment. We want you to feel relaxed and at home during your dental visit at our office. We can help you with your cosmetic and restorative dental needs. We strive to provide you and your family with the most pleasant dental visit ever!

Office Hours
We are open to serve you, answer your questions or schedule an appointment
during the following business hours:

Mondays:        8:00 am - 5:00 pm
Tuesdays:       8:00 am - 5:00 pm
Wednesdays:  8:00 am - 5:00 pm
Thursdays:      8:00 am - 5:00 pm
Fridays:          8:00 am - 12:00 noon

We close each day from 12:15 to 1:45 pm for lunch.

Appointment Policy
Your appointment time is reserved for you. We will make every effort to remind patients by telephone prior to the appointment, but please do not depend on this courtesy.
If we are unable to contact you directly, your appointment card or appointment phone call will serve as confirmation of your appointment and it implies your obligation to be present.

Our answering machine is available for messages left after business hours, however
if a message is left after business hours canceling or rescheduling a next day appointment the patient will be subject to our fee, as this is not considered adequate notice to fill the cancelled/rescheduled slot. We understand that extreme or
unavoidable emergencies or circumstances do arise (ex. death in family, hospitalization), which may require you to cancel your appointment, and individual circumstances will be taken into consideration.

If a patient thinks for any reason there may be a possibility when they schedule an appointment to which they may not be able to keep (ex. scheduled appointment but
still out of town, financial reasons), our office would prefer that the patient wait to schedule when the time is right for them. This will help avoid any fees that can be applied to the account and keep slots open for patients that may be on a waiting list
to be seen.

We reserve the right to charge for any appointment(s) broken without a 24 hours notice. The charge will be a minimum of $35.00 per broken appointment(s) (this charge is subject to increase depending on the length and detail of the appointment that is scheduled).

If you are late for your appointment, we may not be able to accommodate you. If you think that you will be late, we do ask that the patient call as soon as possible so that we may advise you if your late arrival can be accommodated, or if we will need to reschedule you. Habitual missed/cancelled/rescheduled appointments may result in a patient being required to either pay up front prior to scheduling an appointment or this office may no longer be available to provide dental services for the patient.


Dental Emergencies
If you have a dental emergency please call our office immediately at 706.563.5516.
We reserve space during our normal working hours for dental emergencies. If the emergency occurs after hours, call our office number above to hear a recording with instructions and contact information for the dentist on-call.

Some examples of dental emergencies include toothaches, broken teeth,
crowns off, bridges off, severe swelling, or need of a tooth extraction.


Our Payment Policy
Payment of deductible & co-insurance, which is the estimated amount not covered by your insurance company, is required at the time we provide service to you, and in
some cases prior to services being rendered (Our office does require deductibles,
co-insurances, or full payment be collected prior to scheduling such appointments as Periodontics and or any appointment(s) for patients that habitually miss/reschedule/cancel appointments). We will be happy to discuss any financial aspects of your treatment before it is initiated. As a courtesy to our patients, we can also contact your insurance company in advance to get a breakdown of the current dental coverage from the updated/current dental insurance you have provided to us. Upon request, we will gladly submit a claim to your insurance for prior authorization for treatment needed.

We offer a senior citizen discount of 10% to our patients 65 and older.

We accept all major credit cards: Visa, MasterCard, American Express, Discover.


Insurance
We accept most major insurance plans including (but not limited to) Ameritas, Blue Cross Blue Shield, Cigna, Delta Dental, Humana, MetLife, United Concordia.

If you have any questions in regards to your insurance policy (examples: deductibles, co-insurances, what your policy booklet shows as covered or non-covered etc.), and feel that you are not getting a clear answer to your questions from your insurance company, we will gladly try to assist you in helping you to understand your dental policy and coverage.


Dental Financing - "Care Credit"
We offfer a special financing option to all patients who qualify called Care Credit.
This entity accepts applications over the internet and can qualify the applicant while they are in our office. We submit the paperwork and Care Credit handles all terms of repayment. They offer a wide variety of repayment options, typically at low interest
rates amd some periods with no interest. This is a great tool to help patients keep the cost of dental treatment within their budget.
